Instructions. This assessment pertains to the last year—that is, November 1, 2007 through October 31, 2008. Please read each question carefully and check the appropriate answer for each question. Some questions require a written response in a text field. If you fill out this assessment before October 1, 2008, please make an educated guess about your chapter’s actions between October 1 and 31, 2008. Reply to the questions accordingly.
